Aneurin Bevan Health Board…person centred planning
Reach have secured a two year contract working for the Aneurin Bevan Health Board delivering a Person Centred Planning service for people who want to make changes to their lives and make plans for their future.
Carol Eveson, Service Development Manager and Sarah Beckingham, recently appointed as the Person Centred Planning Facilitator, will work on this project over the next two years. Both Carol and Sarah are experienced in Person Centred Planning and Individual Service Design and will be able to bring their experience and knowledge to the project.
Over the two year period we are required to directly deliver a minimum of 25 plans and influence and support the production of a minimum of 15 plans per year. We will also provide training to the staff teams within the Aneurin Bevan Health Board so they can begin to facilitate Person Centred Plans themselves.
This is an exciting challenge for reach. Person Centred Planning is already changing so many lives for the better and this is an opportunity for reach to extend their work and get Person Centred Planning on the map!
